We are in network with many insurance companies but are also happy to see you out of network. The Body Shop will bill any insurance company on your behalf and often your out of network benefits cover our services 100%. It is best to call you’re insurance to verify if we are by simply calling the 800 number on the back of you’re card. Customer service should be able to answer/help with any questions you might have.
Arizona is a direct access state for PT, this means you came always make an appointment for physical therapy without a doctor’s referral. Some insurance companies do require a referral though before they will cover PT services. Medicare and Medicaid require a referral. A deductible is a amount set by you’re insurance that you must meet before the insurance will cover any services billed.
- I.E.: Individual deductible: (calendar year) $500.00
- Family deductible: (calendar year) $1000.00
- Once either is met insurance may/may not cover all expenses. In which co-insurance may apply.
- Co-insurance is a percentage of which the insurance will cover and you (guarantor) will be responsible for a small portion of expenses such as 80/20%. Insurance covers 80% you are responsible for 20%.
A co-payment is a specified sum of money that patients covered by a health insurance plan pay for a given type of service, usually at the time the service is rendered.
What this means is if you have a $20.00 copay, you will be responsible for that amount for each PT visit. This amount can be paid at the time of service or we are happy to bill you for the amount at a later date.
